Exercise training as complementary medicine could mediate the side effects of Paclitaxel chemotherapy in gastric cancer by triggering FNDC5/IRISIN.

Abstract
Introduction: Gastric cancer is often asymptomatic in the early stages of the disease, so it is usually diagnosed late, which subsequently leads to reduced treatment effects and survival rates of patients with advanced gastric cancer. Growing evidence has indicated that Paclitaxel consumption might increase inflammation and oxidative stress. Exercise could be considered as a complementary medicine and non-pharmacological therapy. Exercise training can be effective in the primary prevention of cancers. FNDC5 expression is triggered by endurance training. Exercise training is a well-identified factor that might enhance FNDC5 expression. In addition, the role of FNDC5 in regulating gastric cancer cell proliferation, invasion, and migration was determined.
Methods: We detected the critical hub genes network through artificial intelligence. Moreover, the signaling pathways and important biological mechanisms were identified based on the bioinformatic analysis. Furthermore, the vital biomarkers and hub genes involved in GC were detected. Mice were distributed into Five groups (n=30), including Normal mice, mice treated with N-methyl-N-nitrosourea (MNU) (244 ppm, 18 weeks, GC group), GC mice received Paclitaxel, GC mice received Paclitaxel along with exercise training with moderate intensity, GC mice performed exercise training with moderate intensity. The expression level of the genes was measured via qPCR Real-Time and ELISA methods.
Results: We anticipated a crucial hub gene related to the GC that received Paclitaxel. Data indicated that the expression level of FNDC5 and IRISIN concentration was significantly reduced in the muscles of the GC mice. Moreover, the relative expression of the IL6, IL8, TNFα, Interferon-gamma (IFNγ), NLRP3, and IL1β was enhanced in the stomach of the GC and GC mice received Paclitaxel. In addition, the relative expression of IL6, IL8, TNFα, Interferon-gamma (IFNγ), NLRP3, and IL1β was reduced in GC mice receiving Paclitaxel and with exercise training. GC mice performed exercise training. Notably, receiving Paclitaxel along with exercise training significantly decreased the side effects of the chemotherapy and improved the relative expression of genes.
Conclusion- Exercise training improved inflammation markers and FNDC5 and IRISIN pathways and alleviated the advised effects.
